Job Descrption

Maintain accurate and detailed patient records using our electronic health record systems. Assess, diagnose, and treat patients with a wide range of primary care health issues including urgent primary care presentations. Provide advice and support to patients and their families, ensuring clear communication of treatment plans and arrange appropriate acute onward referral as needed. Participate in clinical meetings and contribute to the continuous improvement of the service.

Able to engage and reflect on feedback and comply with organisational policies with regards to incidents and complaints Keep to time allowing LCW to deliver safe and effective care in time frames to make the patient journey as smooth as possible. The PCCs operate at 15mins appointments with an expectation to complete 4 cases per hour. Be able to supportwith telephone triage in the event workbooks for face-to-face slots are not full.
