This involves reviewing medical records, conducting interviews, and consulting with healthcare professionals involved in the individual's care. You will evaluate the complexity and intensity of their needs to determine if they meet the eligibility criteria for CHC funding Based on assessment findings, make informed decisions regarding the eligibility of individuals for CHC funding. This involves critically analysing the evidence, applying relevant policies and guidelines, and using professional judgment to determine the appropriate funding pathway for each individual. Use clinical expertise to the assessment process. This includes having a sound understanding of various medical conditions, healthcare interventions, and treatment options. Be responsible for providing guidance, training, and mentoring to junior assessors, ensuring they understand the assessment process, eligibility criteria, and documentation requirements. This supports maintaining consistency and quality in the assessment process across the organization. Engage in continuous professional development, attend relevant training sessions, and actively seek opportunities to enhance ones knowledge and skills as a Senior Clinical Assessor. Contribute to the quality assurance process by participating in audits, quality reviews, and feedback mechanisms. Supporting identify areas for improvement in the assessment process and ensures that decisions align with established standards and regulations. Engage with service users, their families, and advocacy groups to gather feedback, address concerns, and communicate CHC processes and decisions. Building meaningful relationships with stakeholders promotes transparency, increases understanding, and ensures that the individual's voice is heard throughout the assessment process Information Management Accurately document assessment findings, eligibility decisions, and justifications in a clear and concise manner. Maintaining thorough records is essential for transparency, accountability, and continuity of care. Planning and Organisation To act as a resource for CHC advice and expertise to Health and Social Care staff with respect to CHC and FNC. Policy and Service Development Adhere to legal and regulatory requirements and ensure compliance with national frameworks and local policies. This includes maintaining confidentiality, following data protection protocols, and upholding professional and ethical standards.
