Join our team as Director of Social Services and foster an environment that enables ourpatients/residents to maximize their well-being and overall qualityof life. You will plan, develop, organize, evaluate and direct theoverall operation of the Social Services department in accordancewith the National Association of Social Workers (NASW) Code of Ethics and federal, state and local guidelines... You may supervise astaff of up to 2 employees.

Position Highlights Collaboratewith social services staff, interdisciplinary team, andadministration to promote and protect patient rights, dignity,independence and psychologicalwell-being

Maintain good working relationshipswith center employees, meeting with administration and nursingstaff to collaborate and plan Assure acomprehensive Psychosocial Assessment is completed for each patientthat identifies social, emotional, psychological needs andstrengths.

Educate patients and familiesregarding their rights and responsibilities, health care decisionmaking/advance directives, effective problem solving, and availablecommunity resources

Qualifications Bachelor's Degree required,preferably in Social Work Must possesscertifications/licensures as required by the state of employment topractice in long-term care.

At least 1-3 yearsof supervised Social Services experience in a health care settingis required.

Additional certification in Geriatric Case Management, Hospice & Palliative Care,Gerontology, Clinical Social Work, Health Care, Nephrology, Mental Health, and/or Substance Abusepreferred.

Management/administrative/supervisoryexperience preferred.
