The Senior Community Health Worker is responsible for helping community residents and Providence Medical Foundation patients and their families to navigate and access community services, other resources, and adopt healthy behaviors. The Sr. Community Health Worker will support in the following areas:
a) Health Education: Assist with recruitment and administrative tasks for health education courses, such as but not limited to Mental Health First Aid, Creating Healthier Attitudes Today, physical activity, or Groceryships. In addition they will deliver these modules these health education topics independently and identify new locations where the programs can be delivered. b) Outreach and Enrollment: Provide outreach in community and PMF settings and provide initial screening for eligibility of Covered CA, Medi-Cal, and CalFresh (Food Stamps). In addition they will provide one-on-one health insurance and/or CalFresh enrollment application assistance. In addition they will also troubleshoot complex cases independently and provide small group presentations on how to use insurance and basic health care rights. c) Navigation/Linkage to Resources: Manage complex patient subpopulations, including but not limited to people with co-morbidities, recently discharged patients from the Medical Centers or community residents struggling with substance abuse.
