The Registered Nurse (RN) Graduate Fellow uses an integrated approach toward achieving desired patient outcomes while ensuring improvements in practice settings by assuming responsibility for self-development in lifelong learning. Provides knowledgeable/caring clinical practice and care coordination through an understanding of patients, families, nurses and healthcare delivery teams. Consistently provides safe/therapeutic care in a holistic and systematic way by incorporating differences into the provision of care.
Job Responsibilities
Performs a comprehensive assessment by interpreting multiple, sometimes conflicting, data and synthesizing their interrelationships. Formulates individualized plans of care based on assessment findings. Anticipates patient variances and makes revisions to plans of care.
Consistently provides safe/therapeutic care in a holistic and systematic way. Incorporates differences into the provision of care. Conducts all patient care in a patient and family centered manner. Uses integrated approaches toward achieving desired patient outcomes.
Provides knowledgeable/caring clinical practice and care coordination through an understanding of patients, families, nurses and healthcare delivery teams. Utilizes standards, guidelines and pathways for care delivery.
Makes clinical decisions based on experience, patient population data and intuition. Promotes clinical decision making development in all team members. Provides validation of clinical decisions for other team members. Seeks validation from others for complex patients or unfamiliar situations.
Supports and empowers patients and families as they progress through the healthcare continuum from health promotion to end of life decisions. Counsels and acts as a resource to resolve issues. Advocates for patients' rights from the patient/family perspective. Assists in assessing patients' learning needs and outcomes measurement of patient education. Anticipates ethical, spiritual and cultural needs while intervening to maximize patient outcomes. Coordinates healthcare and alternate services for patient and family transition planning.
Integrates knowledge, skills and experiences to meet the needs of patients and families throughout the continuum of care to include patient and family education. Interprets trends in patient populations to achieve best practice.
Determines process for delivery of care from direct patient/family communication, technology and other healthcare team member collaboration.
Documents all patient care activities, as per documentation standards, and assists healthcare team members with documentation. Evaluates the outcomes of documentation and contributes to related performance improvement initiatives.
Collaborates with and accesses resources from the multidisciplinary team and incorporates their expertise to achieve desired patient outcomes.
Collaborates in the development and implementation of an educational plan for identified needs. Reviews self-development plan and professional goals and seeks new knowledge toward the progression of professional goals. Demonstrates evidence of self-development by obtaining a minimum of six formal contact hours. Maintains own Team Member Continuing Education Record.
Participates in quality improvement initiatives.
Incorporates the outcomes of unit initiatives or committee work into practice. Incorporates research findings into nursing practice.
May serve as a preceptor following appropriate orientation and competency completion.
Performs other duties as assigned.
Additional Requirements
Certification - Basic Life Support Licensure - Licensed or eligible to practice in the Commonwealth of Virginia as an RN or RN Applicant; the period for RN-Applicant begins the date of program completion and extends 90 days beyond that, whereupon the RN -Applicant must be successfully licensed or revert practice to an unlicensed assistive state (i.e. revert to Nurse Extern 2 role)Experience - None requiredEducation - BSN or AD; if RN has an AD, within six months from the date of hire, they must meet with their nurse leader and conduct the following: 1) Identify which accredited school they plan to attend; 2) Provide a written plan with the anticipated BSN completion date; 3) Submit a review of transcripts from the school indicating the required pre-requisites and timeline for taking the courses; and 4) Complete BSN within 24 months of start date
