Atrium Health Transition Clinic - Cleveland is an outpatient transition services clinic that is committed to helping our patients restore their body and mind. Our Transition Clinic is a multidisciplinary ambulatory Internal Medicine clinic for patients at high risk of complications and readmissions due to medical complexity or barriers to healthcare access. We offer integrated visits with provider, pharmacist, and social worker. During our 30 day follow up period, we identify and alleviate barriers to health focusing on patient engagement, access, and education. Care is delivered in office, virtually and/or with community paramedicine support based on patient’s needs.
