JOB SUMMARY:
Collaborates with Care Coordination staff, other medical center staff and outside agencies to facilitate the smooth exchange of information related to patient care and discharge. Assists professional staff to assure timely, safe and appropriate discharges.
EDUCATION/EXPERIENCE:
High school diploma or GED equivalent. Associate's degree in health related field preferred. Two years office experience preferred.
ESSENTIAL FUNCTIONS:
1. Assists with implementation of a discharge plan by assisting with discharge services including preparing discharge packets, providing current resource lists, initiating FL2s and PASRRs as needed, researching resources to meet identified discharge needs.
2. Arranges concrete discharge services at the direction of the Case Manager and/or Social Worker, including DME, home health services, and transportation (including ambulances, taxis, etc.). Assists case managers and social workers with ALF and SNF arrangements.
3. Contributes to efficient and effective workflow through assigned responsibilities including maintaining clothing closet, assisting with guardianship application process, assisting ED staff, and supporting computer hardware and software needs.
4. Ensures the delivery of Medicare Important Message (IM) letters to all Medicare and managed Medicare patients within two days of patients discharge.
5. Facilitates smooth exchange of necessary information by communicating with Care Coordination staff, other medical center staff and outside agencies.
6. Completes ad hoc reports, filing, data entry and general clerical duties as needed; prioritizes and organizes work in order to ensure timely submission of needed reports at the request of leadership.
7. Assists with audits of case management services and compiles audit findings to reflect metrics that contribute to the evaluation of the quality and effectiveness of services provided.
8. Performs reception activities by responding to questions, assisting customers and answering the telephone in accordance with departmental standards.
