Pharmacist Full Time Days Position Summary
The clinical pharmacist will be a unit-based pharmacist that will verify the orders and provide clinical services for the area(s) for which they are assigned. They will be a drug information resource for the nurses, physicians, patients, and families.
Responsibilities
This individual’s responsibilities include (but are not limited to):
-Order verification and performing medication profile reviews that should include but not limited to: checking for duplications, drug- drug and drug-food interactions, allergies, drug appropriateness, dosing, and frequency.
-Check High Risk IVs, renal dose adjusting medications, IV to PO conversions, initiates/modifies/ discontinues therapy, and mediates black box warning interventions.
-Provide for collaborative practice management as per facility-approved protocols for anticoagulation, parenteral nutrition, vancomycin and aminoglycoside consults, etc.
-Assist with medication reconciliation and clarification of unsafe and unclear drug therapy orders.
Education
Required: Pharm. D. or BS in pharmacy
Experience
Required: 3 years or residency training or equivalent experience
Certification
Required: Pharmacy License; BLS within 30 days of hire;
Preferred: Board Certification in Specialty area, CGP, ACLS, etc.
This individual’s responsibilities include (but are not limited to): All general responsibilities of a Staff Pharmacist. Provide direct patient-centered care and integrated pharmacy services in collaboration with physicians, nurses, and other healthcare professionals. Routine attendance on medical rounds (for decentralized shifts) with consistent documentation of pharmacist interventions. Deliver medication therapy management and ensure pharmaceutical care programs are appropriately integrated throughout the institution. Assist in development and implementation of facility-specific protocols, guidelines, and reference documents related to medication therapy management. Conduct Medication Use Evaluations (MUEs) and assist in making necessary changes to pharmacotherapy practice as needed based on MUE findings.

Required: Pharm.
D. or BS in Pharmacy
Experience:
Required: Pharmacy practice residency or equivalent hospital experience (3 postgraduate years)
Certifications:
Required: Pharmacy license. AHA BLS, ACLS, PALS required if direct patient care responsibilities. Board Certification in Pharmacotherapy (BCPS)
Physical Demands:
Lift/position up to 50 lbs. Push/pull up to 50 lbs. of force. Frequent standing and walking. Frequent reaching, stooping, and bending. Manual dexterity, mobility, touch, auditory to perform all the related duties of the position.
