The Medication Reconciliation team is part of the inpatient pharmacy and staffs 3 RNs and 2 Pharmacy Technicians. Key responsibilities include completing medication histories for admitting patients going through PAT clinic. The ideal candidate must have good understanding of medications, strengths, and purposes. Excellent communication skills with patients, families, providers, nurses, and the pharmacy team, are required.
Shift: 11:30am-11pm Saturday and Sunday

- Collaboration amongst Med Rec team, pharmacist & providers to identify potential issues with transition of care
Qualifications
- Requires an Associates of Nursing (ASN); Bachelor of Nursing (BSN) preferred.
- Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
- Requires that the RN has graduated from a nationally accredited nursing program.
- Requires Basic Life Support certification through the AHA.
- Other advanced Life Support certifications may be required per unit/department specialty according to patient care policies.
