Job Summary
A health coach assesses client lifestyles through patient-reported information and objective health data and works with participants and empowers them to change their behaviours to improve their health and well-being. The health coach is able to work with patients in person and on a digital platform.
Duties and Responsibilities
● Coordinate with the healthcare team ( physician, dietician and diabetes educator ) to create a customized care plan for participants for overall health and well-being in person and in a digital platform
● Understand goal setting and behavioral change processes
● Strategize with participants to understand, set, and update realistic health related goals
● Work with participants to help them reach their health related goals including, but not limited to activity, sleep, nutrition, weight, mood, etc.
● Able to coordinate multiple data points to assess and assist participants in setting and reaching their goals.
● Holds participants accountable in reaching their goals
● Monitor participant progress in achieving their goals
● Motivate participants who are struggling to achieve their goals
● Demonstrate self-confidence and the ability to express views that may not be popular or shared by participants
● Provide coaching in an online format to motivate participants to reduce or eliminate behaviors that are considered high-risk
● Able to understand and follow patient experience metrics
● Distribute educational content as appropriate
● Comfortable communicating in a digital platform
● Computer savvy
● Critical thinking skills
● Languages required: English & Arabic is a positive.
● Ability and willingness to work in a new start up environment where many roles are assumed as the business grows.
● Weekends, flexible shifts will be required within a 48 hour work week
Requirements
● Able to work with minimal supervision in a fast paced environment
● BA/BS degree and minimum desired previous experience in a wellness related field; or an equivalent combination of education and experience (e.g. health/fitness/nutrition educator, community health worker, social worker, and clinical office staff)
● Experience in the healthcare field a positive
● Ability to remain calm under pressure and apply sound judgment while working with high-volume/high-expectation participants
● Excellent interpersonal and customer service skills. Able to maintain helpful, caring and courteous relationships with participants
● Ability to actively listen and communicate effectively. Competent writing skills. Able to present information in a thorough, systematic fashion and perform follow-up routinely
● Ability to manage and oversee multiple tasks simultaneously, ability to prioritize tasks based on degree of urgency
● Excellent analytical and problem solving skills. Ability to develop solutions and recommend changes and follow through with implementation
● Basic knowledge of medical terminology
